Abstract Background Chronic renal failure is a common comorbidity in patients undergoing TAVI. Large amounts of contrast media are generally needed to safely perform TAVI with potential deleterious effects on function and long term life quality. Objectives Every step the procedure can be replaced by no techniques: ultrasound guided vascular puncture final check, alignement 3 guides Valsalva sinuses check coplanarity or cusp overlap, coupling hemodynamics transthoracic echocardiography results need postdilatation, measure gradient across arterial rule out iatrogenic stenosis. Matherials Methods severe (GFR < 30 ml/min/ 1.73 m2) 1 patient moderate previous double transplantation underwent protocol without our center last year. All cases were performed local anesthesia conscious sedation. balloon expandable bioprosthesis self bioprosthesis. Results We able all 4 which technique was attempted complications. Hemodynamics excellent residual significant leaks. No complication occured. Procedural time 20% longer than average but X rays dose exposure reduced because less cine acquisitions needed. Conclusions use feasible safe extensive ultrasound, hemodynamics, multiple depict coronary assure correct prothesis implantation.
